Certified copies of Missouri birth and death records can be
obtained at ACCHU. Birth Certificates cost $15.00 each and
Death Certificates cost $13.00 for the first and $10.00 for each
additional copy of the same record ordered at the same time.
To help protect against identity theft, identification is required
when obtaining a copy of vital records. A document containing
a photo, such as a driver's license, passport, or school, work or
military identification must be presented when applying for a
copy of a vital record. If an applicant does not have a photo
ID, at least two alternate forms of identification must be
presented. The documents must include the applicant's name
and the name of the company or organization issuing the
document.
APPLICATION INFORMATION
To request a copy of a vital record in person or by mail, the
following forms can be used. If an application is being mailed,
the completed form must be notarized.
